COMBINE first 4 ingredients in slow cooker. Cook on HIGH 5 to 7 min. or until bacon starts to brown, stirring frequently. Add next 7 ingredients; mix well. Cover.
COOK on LOW 6 hours (or on HIGH 3 hours). Remove and discard bay leaves. Remove meat from slow cooker with slotted spoon; shred with 2 forks. Return to slow cooker; stir.
HEAT oven to 400ºF. Place crusts on baking sheets; top with meat mixture and cheese. Bake 10 min. or until cheese is melted.
