Mix together everything into crock-pot EXCEPT the cheese, sour cream, parsley and green onion. Make sure the chicken is submerged into the soup mixture.
Cook on high for 3 hours (stirring occasionally). At 2.5 hours, toss in your desired amount of the shredded cheese.
The last 10 minutes stir in the sour cream. Top with fresh parsley and green onions.
